Dive into the pulp: Current concepts in the pulpal treatments of primary and young permanentteeth

  在牙醫師的執業生涯當中,我們常常會遇到如嚴重齲齒、臼齒與門牙礦化不全症(MIH)、central cusp 斷裂、牙中牙、外傷等影響牙髓的狀況,如何最大程度的保存牙髓活性與保留自然牙,是牙醫師必須面對的課題。特別是當我們的病患族群是兒童與青少年時,正確的治療方式可以幫助這些孩子避免缺牙的困擾與日後重建缺牙區的矯正與贋復療程。

  本次的演講學會邀請到馬里蘭大學的James A Coll 教授,為我們帶來最新具有實證醫學的乳/恆牙牙髓治療以及AAPD牙髓治療的新guideline 與 AAPD-AAE有關活髓治療的共同聲明。Dr. Coll從1978年寫了第一篇乳牙根管protocol 開始,就一直醉心於乳牙牙髓治療,疫情前正好完成40年的總回顧。他的思維縝密,邏輯、診斷、治療皆能滿足臨床需求。這次很高興他能再度來台,跟大家分享乳牙牙髓治療的經驗與成果,各位千萬不要錯過!

  下午則由來自泰國清邁大學的兩位副教授,Dr. Papimon Chompu-inwai 和Dr. Tanida Srisuwan 為我們講解如何面對各種年輕恆牙牙髓的問題。Dr. Papimon Chompu-inwai 是美國阿拉巴馬伯明罕大學(UAB)兒童牙科結訓,同時也是美國兒童牙科專科醫學會院士 (ABPD Diplomate)。Dr. Tanida Srisuwan是澳洲墨爾本大學的牙髓病科博士。兩位不同訓練背景的不同專科醫師,不但長期合作發表論文並且攜手到世界各地演講,想必會讓會員收穫良多。

摘要:

The new primary tooth AAPD 2023 Guideline and evidence for vital pulp treatments and AAPD 2020 Guideline for non-vital pulp treatment will be presented. The new proposed 2024 APD-AAE joint recommendations for vital pulp therapy in permanent teeth with reversible or irreversible pulpitis will be highlighted. The participant should be able to diagnose and treat their pediatric and adult patients with the best pulpal evidenced based dentistry.

Dental professionals frequently encounter cases involving young permanent teeth with pulpal involvement. This occurrence can stem from diverse factors such as extensive caries, traumatic events, and developmental anomalies including dens evaginatus, dens invaginatus, palato-gingival groove, and molar incisor hypomineralization. The spectrum of affected teeth encompasses permanent incisors, premolars, and molars. The management protocols for each specific condition exhibit considerable variation contingent upon the underlying etiology, extent of pulpal engagement, and the developmental phase of the tooth. This presentation will comprehensively address these intricate matters, accentuating the respective roles of pediatric dentists and endodontists in the adept management of these complex clinical scenarios.
